Serenity Behavioral Health is committed to providing the highest level of quality care to all our valued patients. Individuals served by Serenity Behavioral Health Systems have online access to insurance and billing, as well as their medical records. Behavioral Health Insurance & Billing
Serenity BHS serves all individuals who are eligible for treatment. Individuals are required to pay a co-payment or fee toward the deductible based on their insurance plan at the time of service. If the individual is uninsured, he/she may qualify for the State Assistance Program. Those individuals will be asked to pay an amount determined on a sliding fee scale, based on income.

Remember to bring your insurance card (CMO, PPO, etc.) with you to each appointment.

Aetna Cigna CMOs (Amerigroup, Cenpatico, Wellcare, CareSource), Humana One, Humana National POS, Humana PPO, Medicaid, Medicaid QMB, Peachcare for Kids, TRI-Care. For additional questions related to insurance, please contact the Billing Manager at 706-432-4837.

Remember to bring your insurance card (CMO, PPO, etc.) with you to each appointment. We have on-site financial staff to help provide support services to individuals and/or their families and agency program staff. Financial staff assist in identifying all available financial resources for which individuals may be eligible, to fund the cost of required services, and improve total quality of life. If the treatment procedure is not covered 100% by insurance, Medicaid, or Medicare, then SBHS will ask for co-payment or a deposit to be paid when the individual checks in for the appointment. A representative from SBHS may contact you prior to the appointment date to make sure all of your information is correct. If your insurance has changed or you need to update your insurance, please fax the updated information to 706-432-3780. Medical Records

Health Insurance Portability and Accountability Act of 1996 (HIPAA) ensures that all medical records are private and that an individual may access copies of their medical records upon completion of a written consent form, also known as the authorization of release of information form. This form is a legal document and is included in the individual’s clinical record. To submit your request, please visit the SBHS Medical Records Department, or you may ask any care representative at check-in when you arrive for an appointment. Release of information forms may also be submitted via FAX to SBHS Medical Records at 706-432-4798. All requests will be processed in the order in which they are received and in a timely manner, but not more than thirty (30) days after receipt of the request (SBHS ref. policy RI 13.01 part IV). Receipt of requests may be mailed, faxed, or picked-up (at a scheduled time).

Record requests are charged by the rates set forth by the State of Georgia Office of Planning and Budget pursuant to O.C.G.A. 31-33-3. The Medical Records Department staff will complete the invoice for medical records retrieval, certification, and copying (SBHS 295), and send to the requestor by fax, UPS service, or pick-up. Fees may be paid by check or money order, payable to Serenity BHS.

1 – 20 pages = $0.97 each page
21-100 pages = $0.83 each page
101+ pages = $0.66 each page

Frequently Asked Questions

No one will be turned away for an inability to pay for services. Please know that our team of Customer Service Representatives, Counselors, Nurses, Nurse Practitioners, and Psychiatrists work closely together to provide you with compassionate and therapeutic care.

Our Adult Outpatient clinic is open to the public beginning at 7:30 AM each weekday and we see individuals on a first-come, first-serve basis (the Child and Adolescent clinic opens at 8:00 AM).

All individuals seeking Outpatient services should bring their Georgia ID or other proof of Residency in Georgia along with any insurance information. It is preferable to also bring Proof of Income and your Social Security card. For Children and adolescents please see our C&A intake form and fill it out to bring with you at your first appointment. If you would like assistance with filling it out, please call our C&A outpatient clinic.

A referral is not required; however, we ask that if applicable, please bring any hospital discharge summaries or pertinent records from any recent treating provider.
Any individual that comes to Serenity in crisis, will be seen, regardless of the state they live in. For our Outpatient clinics, individuals must reside in the state of Georgia.
You are welcome to bring loved ones for support during your visit with us in addition to your ID and Insurance information.
